Implicações cardiovasculares ao paciente acometido pela COVID-19: revisão integrativa

Objetiva-se identificar as implicações cardiovasculares no paciente adulto acometido pela COVID-19. Trata-se de uma revisão integrativa, buscando-se analisar e sistematizar publicações científicas sobre o tema. A seleção ocorreu nas bases de dados MEDLINE, LILACS, IBECS e BDENF, através do indexador Biblioteca Virtual em Saúde (BVS), com os descritores “Infecções por Coronavírus” e “Doenças Cardiovasculares”. Encontraram-se 617 artigos, sendo 1 duplicado. Estabeleceram-se os critérios de exclusão: teses, dissertações, relatos de casos, estudos de revisão e que não respondessem à questão da pesquisa. Quatorze estudos atenderam aos critérios e demonstraram que pacientes críticos com COVID-19 e doenças cardiovasculares apresentaram risco de mortalidade significativamente maior que pacientes não críticos. Identificaram-se as seguintes implicações cardiovasculares em pacientes com COVID-19: cardiotoxicidade direta mediada pelo vírus, lesão relacionada à hipóxia, tempestade de citocinas imunomediadas, inflamação sistêmica, infecção do pericárdio, apoptose, entre outras. Tal observação subsidia protocolos clínicos que antevejam esta deterioração clínica mais rapidamente.

Cardiovascular implications for patients affected by COVID-19: integrative review
Abstract: The objective is to identify the cardiovascular implications in the adult patient affected by COVID-19. This is an integrative review, seeking to analyze and systematize scientific publications on the subject. The selection took place in the MEDLINE, LILACS, IBECS and BDENF databases, through the Virtual Health Library (BVS) index, with the descriptors “Coronavirus Infections” and “Cardiovascular Diseases”. 617 articles were found, with 1 being duplicated. Exclusion criteria were: theses, dissertations, case reports, review studies and those which did not answer the research question. Fourteen studies met the criteria and demonstrated that critically ill patients with COVID-19 and cardiovascular disease had a significantly higher mortality risk than non-critical patients. The following cardiovascular implications were identified in patients with COVID-19: direct virus-mediated cardiotoxicity, hypoxia-related injury, immune-mediated cytokine storm, systemic inflammation, pericardial infection, apoptosis, among others. The observation supports clinical protocols which anticipate this clinical deterioration more quickly.

Implicaciones cardiovasculares para pacientes afectados por COVID-19: revisión integradora
Resumen: El objetivo es identificar las implicaciones cardiovasculares en el paciente adulto infectado por COVID-19. Esta es una revisión integrativa, que busca analizar y sistematizar las publicaciones científicas sobre el tema. La selección se realizó en las bases de datos MEDLINE, LILACS, IBECS y BDENF, a través del índice de la Biblioteca Virtual en Salud (BVS), con los descriptores “Infecciones por Coronavirus” y “Enfermedades Cardiovasculares”. Se encontraron 617 artículos, pero 1 era duplicado. Se establecieron criterios de exclusión: tesis, disertaciones, informes de casos, estudios de revisión y aquellos que no respondieran a la pregunta de investigación. Catorce estudios cumplieron con los criterios y demostraron que los pacientes críticos con COVID-19 y enfermedad cardiovascular tenían un riesgo de mortalidad significativamente mayor que los pacientes no críticos. Se identificaron las siguientes implicaciones cardiovasculares en pacientes con COVID-19: cardiotoxicidad directa mediada por virus, lesión relacionada con hipoxia, tormenta de citocinas inmunomediada, inflamación sistémica, infección pericárdica, apoptosis, entre otras. Tal observación apoya los protocolos clínicos que anticipan más rápidamente este deterioro clínico.
